New Patient Registrations

Registering with Us

If you are new to the area and are requiring registering with the practice, please ask at our Reception; if you have your Medical Card, then please bring this along with you.

Following your registration, you will be required to have a new patient check to enable your registration to be fully complete; please ensure the form is fully completed as incomplete forms will not be accepted.

You will be sent a SMS to arrange a new patient health check after which you can book appointments and order medications; please ensure your contact numbers are up to date.

We do not refuse patients based on race, gender, social class, religion, sexual orientation, appearance, disability or medical condition.  We accept patient in our catchment area, whilst our list remains ‘Open.’

The Practice would normally refuse registration to patients who:

  • Reside 3 miles outside the publicised Practice are of Handsworth.
  • The patient has been previously removed from the list.
  • The patient has a known history of violence.

Non-urgent advice: Zero Tolerance

We operate a ZERO TOLERANCE POLICY. If you are violent or abusive in any way towards our staff, we have a right to remove you from the premises and practice list.
